Your First Visit

Once you call to make an appointment (239-437-8000), you will be emailed or mailed your New Patient Packet.

Ideally, the paperwork will be received to APMSS prior to your scheduled first appointment.

You should bring the completed New Patient Packet to your first office visit. Otherwise, please plan to arrive 30 minutes prior to your appointed time to complete the forms.

All Patients (including New Patients) – Please bring the following to every appointment:

  • Valid driver’s license
  • Health insurance card (as well as the insurance card for secondary insurance, if applicable)
  • All X-rays and radiologic reports
  • All medications you are taking in their bottles
  • Money or credit card to cover your full co-pay

Please note:

If you are going to be more than ten minutes late, call the Front Office at 239-437-8000 to inform the staff. You may need to reschedule.

Although we realize unexpected things can occur, we reserve the right to not reschedule new patients who do not show up without prior notice.

First Time Auto and WC patients:

If you are an auto accident patient, your records must already be at APMSS to be seen. But please also bring the items required for new patients as well as:

  • Accident Report
  • Insurance Carrier with contact info

If you are a Workers’ Comp patient, please refer to the Workers’ Comp section.

Our staff is great, but it is beyond their talents to answer questions about someone who has not been seen by our doctors. We highly recommend that a person suffering from chronic pain or acute pain make an appointment and be evaluated thoroughly by an interventional pain physician.
