It is helpful when patients can return their completed paperwork prior to their first scheduled appointment.
If you cannot send your completed New Patient Packet prior to your appointment, bring it to your first office visit. In this case, please arrive 30 minutes prior to your appointed time to complete the forms and allow us to process them.
If you are going to be more than ten minutes late, call the Front Office at 239-437-8000 to inform the staff. You may need to reschedule.
Although we realize unexpected things can occur, we reserve the right to not reschedule new patients who do not show up without prior notice.

Our staff is great, but it is beyond their talents to answer questions about someone who has not been seen by our doctors. We highly recommend that a person suffering from chronic pain or acute pain make an appointment and be evaluated thoroughly by an interventional pain physician.
