If you are considering becoming a patient at our practice, please call 239.437.8000 to make an appointment. Provide our staff with your mailing address or fax number and we will provide you with the necessary forms to complete for your first appointment. We apologize for the inconvenience to you, and we plan to have the forms added to our web site shortly!



Questions about becoming a patient?

How much time should I allow for my first appointment at your practice?
It depends. If you have completed the new patient's forms before your appointment, then please arrive at least twenty minutes ahead to Check-In. If you have not completed all the forms, you must arrive at least one hour before your appointment. For all established patients, you should arrive at least ten minutes before your appointment. New patients require more time.

What should I bring to my first appointment?
At Check-In, you are required to bring your current insurance cards, your driver's license, and any medical records and reports that may not have been sent to our office before your visit. Your insurance card will be verified and copied, your driver's license will be copied, and your picture will be taken for your medical record.

We are required by law to collect your co-pay, so please bring the funds to pay it at the time of your first visit as well as all subsequent office visits. Your insurance co-pay will be collected before you seen by our clinical staff. New patients are required to pay their co-pays by credit card or cash.

What should I do if I realize that I will be late for a scheduled appointment?
We understand that patients may be plagued by unexpected problems that may cause them to be late for an appointment. If you know you will not be able to arrive on time, please call the office where your appointment is scheduled. It is not fair to patients to who have arrived on time and our staff, to keep them waiting if you are going to be late.

New Patients:
If you are running late for your first appointment, it may be a problem. If you do not come with the completed patient forms, it may not be possible to seen that day.

Current Patients:
If you are established patient and you are going to be more than ten minutes late for a follow-up visit or a medical management visit, you may also be given the option of either waiting to the end of the day to be seen or rescheduling your appointment. Patients who are chronically late need to reschedule.

What happens if I do not cancel an appointment?
If you call the office at least twenty-four hours in advance and reschedule an appointment, there is no charge. If you do not call and fail to show up, you will be charged a "No Show" fee. You are charged $50.00 for missing an office visit, and $100 for missing a procedure.

The "No Show fee" and your co-pay must be paid at Check-In at your next appointment.
